Attrition, abrasion, corrosion and abfraction revisited: A new perspective on tooth surface lesions
Although there are known situations where only one single process causes tooth wear (e.g. nocturnal teeth grinding produces wear by attrition alone), the clinical presentation often results from a combination of tooth wear processes. When different tooth wear mechanisms act contemporaneously, diagnosis of dental erosion can be difficult (O’Sullivan & Milosevic 2007).
Attrition is caused by grinding and clenching your teeth. Patients who grind their teeth at night typically cause the most damage. The affects of nightly grinding or bruxism can be greatly reduced by wearing a night guard while you sleep. Abfraction lesions are V-shaped while the abrasion and attrition lesions are flat. A chemical process is involved in erosion, unlike the other three which are mechanical in nature. When the acid level in the saliva rises, it begins to dissolve the enamel.
Tooth tissue is gradually weakened causing tissue loss through fracture and chipping or successively worn away leaving a non-carious lesion on the tooth surface. The etiologies of abfraction, abrasion, attrition, and erosion may be interrelated. Therefore, multiple conditions may be seen in a single patient ( Figures 17-1A to C Figure 17-1A to C: These photographs illustrate the complex dental condition of a 71-year-old male: (A) The palatal cervical regions of the maxillary anterior teeth exhibit sharp, wedge-like lesions that are characteristic of The Herman Ostrow School recently published the following infographic that includes definitions, causes, and ways to protect teeth from erosion, attrition, abrasion, and abfraction. Learn how to diagnose, monitor, and treat different types of tooth wear with this infographic.
The next most common cause of tooth wear is attrition. Attrition is caused by grinding and clenching your teeth. Patients who grind their teeth at night typically cause the most damage.

Tooth wear is the result of three processes: abrasion (wear produced by interaction between teeth and other materials), attrition (wear through tooth-tooth contact) and erosion (dissolution of hard tissue by acidic substances). A further process (abfraction) might potentiate wear by abrasion and/or erosion.

Tooth wear may present as abfraction, abrasion, attrition, and erosion. It is well established that the most common cause of attrition is bruxism.

Unlike dental attrition, abrasion is the loss of tooth structure by mechanical forces from a foreign element. If this force begins at the cementoenamel junction, then the progression of tooth loss can be rapid since enamel is thin in this region of the tooth.
